Our main (Dallas) office is open Monday through Friday from 8:30 AM to 5:00 PM. If you have an urgent issue after our regular business hours, you may call our main office at 214.345.4160 and our answering service will page the physician on call. Non-urgent messages will be handled by our staff the following business day.

Appointments with Dr. Lam can be scheduled by calling our main office at 214.345.4160. Whether you are a new or returning patient, please bring your insurance card with you at the time of your visit. If you are a returning patient and your insurance policy has changed, please notify our office prior to your visit so that we can verify our participation in your health plan.
Items to Bring:

- List of all medications

- Name and contact information for primary care physician
- Name and contact information for any previously seen specialists
Providing you with exceptional care is of utmost priority to the Lam Vascular & Associates team.
Depending on the severity of your case and the number of tests needed to diagnose your condition, appointments can take up to two hours or more. It is best to plan accordingly. This is in an effort to perform all tests in one day, although this is not always the case.
We support our patients having access to their health information through the Patient Portal.
Beyond the Patient Portal, we can enable certain mobile apps that meet requisite technical specification and security requirements to provide you access to your health information in a safe and secure way. Currently, no mobile apps have been configured to connect to our system. Our website, www.lamvascular.com, will be updated if any additional apps become available for you to use to access your health information. Please contact our office if you have questions.
For your convenience, you can click here to download new patient paperwork. Please print out the paperwork, and use blue or black ink to fill it out. Bring the completed paperwork with you to your first appointment.
If you are unable to complete paperwork before your appointment, please arrive at least 15 minutes before your scheduled time to fill out all paperwork before being seen.

Our office will file your insurance claim directly to your health insurance carrier. All copays are due at the time of service. Our office accepts cash, personal checks, money orders, Visa, MasterCard, and American Express. If you have questions regarding your account, please contact our main office at 214.345.4160.
Disability Forms/FMLA Forms
If you have a disability or FMLA form that needs to be completed by our office, please allow five business days for processing. There is a $15 fee for each form. The forms can be mailed, faxed or picked up upon receipt of payment.
Medical Record Requests
Medical record requests must be made in writing and authorized by the patient. All record requests will be processed within ten days of the written request. All medical record requests are handled in our main office.
Please call your pharmacy for any refill requests. The pharmacy will contact our office with any questions regarding your request. Refill requests will not be processed after regular office hours or over a weekend.
