The Dallas, TX, office is at Texas Health Presbyterian Hospital on the corner of Walnut Hill and Greenville Avenue, across from Bank of America. The office is located on the fifth floor (suite 505) in Professional Building 1. Once in the building, take the elevator to the fifth floor. Take a right off the elevator and then another right. You will see the glass doors and the Lam Vascular logo.
Parking
Once you see the hospital, look for signs to guide you to Professional Building I. Depending on your preferred parking location, directions are below.
Valet
- Available 8:00 a.m. to 5:00 p.m. in front of Professional Building II
- This option costs $7.00 regardless of time spent on site
- If needing vehicle after 5:00 p.m., security can be called
- Wheelchairs are available if needed for transport to the fifth floor
Lot 3
- Covered self-parking available for $2.00 for all day (pay by cash or check)
- $1.00 for all-day parking for people over 65 with sticker (sticker can be obtained at the Volunteer Desk on the 1st floor, between Professional Buildings I & II)
- To access the lot, stay in the far right lane
- To access the building, use the crosswalk to the entrance of the Professional Buildings
- Parking will be validated for surgery patients
Lot 5
- Uncovered self-parking available for $2.00 (pay by cash or check)
- $1.00 discount for 65+ also applies with sticker (see above for details)
- Located between Professional Buildings II & III
